The Black Cats will be away to whoever finishes fifth in the Championship in the first leg on Friday, May 9, with the second leg then taking place four days later on Tuesday, May 13 at the Stadium of Light.
Both games will kick off at 8pm.
Ticket details for the first leg will be announced in due course, following this weekend’s final day fixtures, but tickets for the home leg are now on sale.
Season ticket holders can secure their seats for what Sunderland have described as an exclusive loyalty price.
The offer will be available until next Tuesday, May 6, after which loyalty pricing will expire, general sale pricing will be introduced and all seats will be released.

Tickets are available online or in person at the Stadium of Light ticket office, which is open from 10am-5pm Monday to Friday. The ticket office will be closed next Monday, May 5 for the Bank Holiday.
Due to the short turnaround before the game, tickets will not be available to buy over the phone.
As things stand, Bristol City are fifth in the Championship. The Robins host Preston and will set up a semi-final clash with Sunderland if they beat struggling North End on Saturday.
